Theresa Diane Moore, 62, a resident of Cleveland, TN, passed away on May 5th, 2016 in a local hospital.
She was preceded in death by her father, Grady Smith.
She leaves behind to cherish her memory, her son Michael Gann and two grandchildren, Braden and Savanna Gann.
A memorial service will be held at 4:00pm on Saturday May 14th, 2016 at Companion Funeral Home. Visitation will be held from 3:00pm until the 4:00pm service time.
